WHERE DO I FILE THE ANNUAL STATEMENT AND PREMIUM TAX PAYMENT?

In January, the Stamping Office will send a notice to each broker of total tax due for the previous year's submissions.

The original Annual Premium Tax Statement form and tax payment is sent to the Insurance Department by the licensed SL Broker. Tax payment is made in full annually on or before March 1.

A $25.00 per day penalty is assessed by the Department of Insurance commencing April 1.
