INDEPENDENTLY PROCURED TAX REPORT & INSTRUCTIONS This form is strictly for transactions that do not include a broker, agent, or producer. The insured must purchase direct from the insuring company.
BROKER MEMBER-LICENSEE REPORT This form must be completed as soon as the broker is licensed in Idaho.
ANNUAL S/L PREMIUM TAX STATEMENT No later than January 31 of each year, we will send you correspondence itemizing accumulated premium reported for the previous calendar year and premium tax due the Idaho Department of Insurance. We will also include the form, Statement of Premium Taxes, which you will complete and send to the Idaho DOI. The Annual Statement must be submitted to the Idaho Department of Insurance even if your tax consequence is "0." If you were licensed in Idaho as a SL broker during any portion of the tax year but your license is no longer active, the form still must be submitted.
